Fall sports practice will begin August 6th for high school and August 13th for middle school.
CHSAA is implementing ArbiterAthlete and requiring every member school use this service for reporting purposes. This will require every student athlete and one parent to create an ArbiterAthlete account. (see below "kick off letter" for instructions). Required athletic participation forms will be completed online through your ArbiterAthlete account. ALL REQUIRED FORMS ARE ON ARBITERATHLETE. Student athletes must have a current physical and parent permission on file before they will be allowed to participate.
The Physical Evaluation Form completed by a doctor can be uploaded to ArbiterAthlete by taking a picture of the document or it can be turned in at the hs/ms office.
The Physical History Form can be completed on ArbiterAthlete or it can be turned in at the hs/ms office with the physical.
All other permission forms (including the parent permission) should be completed through your ArbiterAthlete account.
STUDENT ATHLETES SHOULD CREATE THEIR ACCOUNT FIRST. You will be prompted to link your account to a parent - you must have an email or mobile phone number for your parent at this time. This will send an invitation to your parent to create their account.
PARENTS WILL NOT BE ABLE TO SEE THE LIVE FORMS UNTIL THEIR ACCOUNT IS LINKED TO A STUDENT.
You MUST select the correct school - look for "Dove Creek High School" or "Dove Creek Middle School"
DO NOT USE "Dolores County School District RE-2J"
Students and parents will both be required to sign the documents from their own account.
