Fall sports practice will begin August 12th, 2019.
In 2018 CHSAA implemented ArbiterAthlete and has required that every member school use this service for reporting purposes. This will require every student athlete and one parent to create an ArbiterAthlete account. (see below "kick off letter" for instructions if you have not yet created an account). If you created an ArbiterAthlete account last year you DO NOT need to create a new account, you must use your existing account. ALL REQUIRED FORMS ARE ON ARBITERATHLETE and should be completed electronically. These forms must be completed annually. Student athletes must have a current physical and parent permission on file before they will be allowed to participate in practice.
A blank Physical Evaluation Form can be printed from arbiterathlete (click on "athletic forms", "Example forms") Once signed by a physician, this document must be uploaded to ArbiterAthlete under "CHSAA Physical Evaluation Form"
The Physical History Form can be completed on ArbiterAthlete or it can be uploaded by taking a picture of the document.
All other permission forms (including the parent permission) should be completed and signed electronically on ArbiterAthlete.
STUDENT ATHLETES SHOULD CREATE THEIR ACCOUNT FIRST. You will be prompted to link your account to a parent - you must have an email or mobile phone number for your parent at this time. This will send an invitation to your parent to create their account.
PARENTS WILL NOT BE ABLE TO SEE THE LIVE FORMS UNTIL THEIR ACCOUNT IS LINKED TO A STUDENT.
You MUST select the correct school - look for "Dove Creek High School" or "Dove Creek Middle School"
DO NOT USE "Dolores County School District RE-2J"
BOTH STUDENT AND PARENT ARE REQUIRED TO SIGN ALL DOCUMENTATION ELECTRONICALLY FROM THEIR INDIVIDUAL ARBITERATHLETE ACCOUNT.
